2010 USA Swimming Registration Instructions
All swimmers, returning and new must complete a 2010 USA Swimming Registration Form. NO EXCEPTIONS!
If we do not receive a form your swimmer will not be allowed to participate in practice, at a meet or attend team social activities.
COMPLETED FORMS AND ANY REQUIRED PROOF OR NAME AND AGE SHOULD BE TURNED IN AT THE TIME OF SWIM TEAM REGISTRATION.
Please print and complete a Registration Form;
1. Complete ALL areas of the form - Swimmer Info, Parent info, Citizen info, Disability info (if applicable) and Race & Ethnicity info. USA Swimming has become very strict in this area.
2. The Club Code is CAA & the Name of the Club you Represent is Columbia Aquatics Association - this has been entered for you on the form linked above.
3. The form must be signed by the swimmer's parent or legal guardian.
4. There are NO fees due with the registration. The Registration Fee of $70 is paid on behalf of the swimmer by CAA and charged to your Escrow Account.
5. Swimmers NEW to the Clippers who HAVE NOT been previously registered with USA Swimming, must include a copy of a birth certificate, proof of birth, passport, military ID or baptismal certificate with birth date and legal name.
6. Swimmers New to the Clippers who HAVE been previously registered with USA Swimming through another team, must indicate the prior team's Club Code, LSC Code, and date of last competition.
7. Completed forms and any required proof of name and age should be turned in at the time of Swim Team Registraiton
