2009 USA Swimming Registration Instructions
All swimmers, returning and new must complete a 2009 USA Swimming Registration Form no later than September 10 or within 15 days of joining the team. NO EXCEPTIONS! If we do not receive a form for your swimmer by the deadline stated above, your swimmer will not be allowed to participate in practice, at a meet or attend team social activities.
1. Complete ALL areas of the form. We will not process incomplete forms! USA Swimming has become very strict in this area.
2. The Club Code is CAA
3. The Name of the Club you Represent is Columbia Aquatics Association
4. The form must be signed by the swimmer's parent or legal guardian
5. Swimmers who have not been previously registered with USA Swimming, must include a copy of a birth certificate, proof of birth, passport, military ID showing birth date and legal name, or baptismal certificate.
6. Submit the completed form and any required proof of name and age to the CAA Locked Box at the Swim Center or mail to the Aquatics Office at 9450 Geriwg Lane Columbia MD 21046; attn: Clippers.
7. There are NO fees due with the registration. The Registration Fee of $65 is paid on behalf of the swimmer by CAA and charged to your Escrow Account.
